Welcome to the 2022-2023 School Year.

I am Mrs. Mattei, and I will be your child's school nurse.  

Some important reminders:

Students entering Grade 6 must show proof of immunization (Tdap and Meningococcal).  Documentation should be submitted via email or placed in my mailbox.

Any medication that may be required during school hours or during a school-sponsored activity must have a completed healthcare provider medication authorization on file each school year.  Students may be authorized to carry their inhaler and/or epinephrine autoinjector if their healthcare provider makes that indication on the required medication authorization form.  Medications are stored in the school health office and must be brought to the school nurse by the parent/guardian along with completed medication authorization forms.  Medications must be in pharmacy bottles (prescription) or in the original, unopened packaging (over-the-counter). 

For those students interested in school-sponsored athletics (Grades 6-8):  A sports physical is required.  It is suggested to have your child's healthcare provider perform this physical during the annual well-child visit.  Forms are available online.  Sports physicals are valid for 365 days from the date of the exam.  All sports physicals must be reviewed by the school district physician before participation can be approved.
