Upcoming Season Information

  • In preparation for SPRING SPORTS - Below addresses the physical exam requirement for school-based sports.  Please review the following information and dates with regard to Sports Physicals.  Students cannot participate without a cleared sports physical.  
    Signing up for sports is a separate process and information will be communicated at a later date. 
    Students who will be in Grades 6,7 and 8 in current school year are eligible for school sports at ABIS.
    • A valid NJSIAA Sports Physical Exam is the only physical exam accepted for school-based sports.  The physical is considered current for 365 days from the date of the physical.  
    • Forms are available online and are also available in print outside the ABIS Main Office. 
    • Parent/Guardian: The history questionnaire is required to be completely filled out prior to the physical.  Any "Yes" answers should be briefly explained in the space provided on the form.   The Form Must Be Signed and Dated in INK.  Incomplete forms cannot be accepted.
    • Sports Physicals should be conducted by the student's medical provider.  Be sure the examining provider has signed, dated and stamped the form where indicated.  Examining providers must be able to sign-off  on the cardiac education module, please check they have completed this section prior to submitting forms (bottom of last page).  If your child's provider is performing the physical - the completed physical needs to be submitted by the due date outlined below.
    • A limited number of spaces are available for students who do not have a medical home and are in need of a school-based sports physical.   If your child requires the school MD to perform their physical, the completed paperwork (questionnaire) by the due date outlined below.
    • Students who need an inhaler for the treatment of asthma, or an epinephrine auto-injector for anaphylaxis MUST HAVE COMPLETED MEDICATION AUTHORIZATION FOR THE CURRENT SCHOOL YEAR ON FILE.  A student cannot be cleared if they do not have their required medication authorization.  Forms should be filled out by the medical provider, signed by the parent and submitted directly to Mrs. Mattei.

    Deadline to turn in paperwork for SPRING Sports Physicals
    • Students needing the school MD to perform the physical: February 1, 2024
    • Private MD providing the physical:  February 15, 2024
    Sports physicals must be reviewed and approved by the school district physician.  Please ensure any paperwork submitted is complete to avoid a return of forms that may cause a delay.

  • Students with Asthma or Allergies

    If your child requires the use of an inhaler to treat asthma or an epinephrine auto injector for the treatment of anaphylaxis: Medical Authorization for the 2022-2023 School Year Must Be Submitted with the sports physical. 

    Tips for Completing the Forms

    The Pre-Participation Health Questionnaire must be completed with every question answered. Any "YES" answer must be explained in the space provided. The student athlete and the parent must sign and date this form. The examining medical provider must review this form at the time of the physical exam.

    Medical providers performing the exam need to complete all areas. BE SURE TO CHECK that the provider has signed both sides of the exam, Has Stamped the Form with their Office Stamp and has signed and dated the cardiac module completion that is located on the back of the exam form.
